If you lose your learner permit then you need to fill out a form called D201. This can be obtained in any Garda Station, NDLS centre, Test Centre or Theory Test Centre. This document needs to be stamped and signed by a Garda.
This form must be brought in person to the local NDLS centre where they will take a photo of you and request you make a digital signature for your Permit. This is to enhance security and protect against fraud. You will also have to bring evidence of your PPSN number, and proof of ID. You must also bring a medical report if this is required in your case. The fee is €35.00.
If you lost Learner Permit has expired then you can only apply for a new Permit, it still costs €35.00 and must be stamped and signed by a member of the Guards.
You should always carry your Permit or full License on you when driving.
